Your options for endometriosis treatment.
Patients have the option of three main methods to manage their endometriosis: observation; palliation, or symptom management; and surgery. Endometriosis excision surgery is the most effective way to treat endometriosis.
Excision of endometriosis is the focus of the center, but our team takes the time to explain your options for managing endometriosis, enabling you to make an informed choice.
Observation
Observation of endometriosis is one of the treatment options, where a patient can choose to not treat their disease and just deal with it and watch it. After an appropriate staging ultrasound, some patients will choose this option and plan follow up exams and staging to monitor for disease progression.
Consultations assess each patient’s unique goals, expectations, and preferences. For some patients, observation is the only treatment method they want to receive.
Palliation
Palliation is the management of symptoms. The goal of palliative care is to minimize pain and discomfort, but it does not treat the underlying endometriosis. A multi-disciplinary approach to palliation is ideal to support your health and offer the most symptom relief.
Palliation may include:
- Hormones such as birth control pills, progesterones, IUDs, etc.
- Other medications or dietary supplements
- Changes to diet
- Exercise
- Stress management
- Physical therapy
- Chiropractic care
- Referral to a complex pain management practice
- And more.
Surgery
There are different types of surgical management of endometriosis. The gold standard, which we specialize in at EndoWest, is excision surgery.
When an individual has ovarian endometriosis, endometriomas can occur, forming cysts inside the ovary. The goal of excision surgery is to remove the cyst and the cyst wall, avoiding the removal of normal ovarian tissue. This has a lower likelihood of recurrence compared to other surgical options.
We take this same approach to all areas of endometriosis. By performing wide excision of concerning areas, we feel we can limit the risk of missing or undertreating disease and provide a more complete removal of the endometriosis.
Excision surgery can help you:
- Improve or end pain
- Protect fertility
- Limit progression
- Support an improved quality of life
- Provide the ability to pursue life goals
We partner with you, discussing all potential treatment options, so that you can get the right medical treatment for you.

Get your endometriosis excision surgery with a leading authority.
Dr. Jeff Arrington is an experienced surgeon who is an outspoken advocate for the treatment of endometriosis. You can rely on him to keep you informed, explain your options, and give you exceptional care.
With more than two decades of experience and a specialization in endometriosis excision surgery, Dr. Arrington is an exceptional surgeon who focuses on empathetic medical care. He always works to challenge his own skills and knowledge rather than be complacent or allow patients to deal with a condition they have been told is hopeless.
Repeat surgeries for endometriosis are to be avoided, and Dr. Arrington knows the importance of the first surgery for endometriosis. Each repeat surgery to remove endometriomas increases the damage and makes it harder to ensure appropriate and complete treatment.
Less than 20% of Dr. Arrington’s patients need repeat surgeries. He and the clinic focus on excision techniques that remove the full disease and identification of all forms of endometriosis in a patient. We believe this effort allows our excellent results.

What to expect in an initial consultation.
In an initial consultation, either in-person or using telehealth, our goal is to:
- Listen to your situation
- Explain what we suspect about your condition
- Provide you with information on your options for treatment
- Help you make an informed choice about your next steps
We walk through your experience with endometriosis, how it affects your life, and your goals for treatment.
Your unique situation is important
It is important to us that you are able to outline the impact of your condition and what you want from treatment. This might include being able to engage in your work or education with less interruption, to be intimate without pain, to avoid rearranging your life around your periods, to prevent or improve endometriosis related infertility, and other goals.
Share as much as you can
It is most helpful when you are able to provide as much information as possible during your consultation. This includes:
- Details on your symptoms
- Medical information
- Prior diagnoses
- Other relevant information
At EndoWest, our goal is to explain the information we have based on your symptoms and any physical exams so that you have clarity on your condition. We want to give you the support, certainty, and relief you need during this stressful time.
Book a ConsultationEvaluation of your endometriosis.
Endometriosis occurs when cells similar to but not the same as uterus lining cells are found elsewhere in the body. It can have many symptoms or very few, and the symptoms are not necessarily related to the severity of the disease.
To properly evaluate endometriosis, where it may be present in the body, and the impact it has on your life, all the potential symptoms are relevant.
Some symptoms of endometriosis include:
-
Severe menstrual period pain in the pelvis, often preventing you from going to school, work, or engaging in daily tasks
- Lower back pain
- Pain during sexual intercourse, usually from deep penetration
- Blood in stool during a menstrual period
- Constipation, diarrhea, and a diagnosis of irritable bowel syndrome (IBS)
- Painful bowel movements
- Frequent and/or painful urination
- Bloating
- Fatigue
- Infertility
-
Signs of a higher risk. If you missed school due to period pain, or family members have a history of endometriosis, or you have taken hormones, like birth control pills, to address your period pain, these are signs of a higher risk of endometriosis.
Making appointments for further evaluation.
By listening to a patient describe their symptoms, we can know if endometriosis should be considered. Often, we can get an idea of what areas are actually involved. After gathering the patient’s personal history, we can then direct a symptom focused physical exam and any necessary imaging to get a better understanding of what stage the endometriosis may be. This may include:
-
Ultrasound & UBESS
An ultrasound or transvaginal ultrasound using a modified Ultrasound-based Endometriosis Scoring System (UBESS) done during your consult for immediate feedback
-
MRI
Magnetic resonance imaging (MRI)
-
Pelvic exam
A pelvic exam
-
Laparoscopy
Laparoscopy (usually combined with treatment at the same time)
These tests are meant to determine if there are any masses or cysts suspicious for endometriosis or adhesions (tissue connections between organs) that result from the endometriosis-caused inflammation.
These tests may be used either during initial diagnosis or in preparation for a potential surgery, but a list of your symptoms can be sufficient to get near a diagnosis.
Planning the next steps of your treatment.
With our team, we always discuss the benefits and drawbacks of these tests for your treatment, your future care, and your diagnoses. The final decision for whether or not to complete an evaluation is up to you, although many are a key part of any treatment plan.
Observation alone cannot address your symptoms or stop endometriosis. The disease can continue to progress during this time which can lead to additional or worsening symptoms, including a greater risk of infertility.
It is up to every individual patient whether they wish to take further steps beyond observation. If you have minimal symptoms, less-severe endometriosis, and no concerns about future fertility, you may not want additional palliative care or surgery.
However, you must know that progression of endometriosis can lead to more severe outcomes and become more difficult to fully treat.
A major part of observational endometriosis treatment is monitoring changes in your symptoms and your disease.
There are four stages of endometriosis, classified based on the location and depth of endometrial implants, meaning the spots where the tissue is growing outside the uterus.
- Stage IMinimal, superficial implants
- Stage II
- Stage III
- Stage IVSevere, deep implants
Stage I involves a minimal amount and superficial depth of implants, while Stage IV involves severe and deep implants, large cysts on the ovaries, and dense adhesions, and sometimes bowel involvement.
The stage of your endometriosis may not correlate with your pain or symptoms. You might be dealing with a serious stage but limited pain, or severe pain at a lower stage. A higher stage of endometriosis carries other risks, bowel obstruction, ureter closure resulting in kidney damage, scarring or disease around nerves, among others.
Our team helps you understand how your endometriosis changes over time, and tells you the risks and drawbacks of different treatment approaches depending on the severity of your condition’s stage.
This option is essentially the decision to continue without treating disease or symptoms. It does not verify the suspected diagnosis and one should know that, if there is endometriosis, it may continue to grow. It is important to have a plan in place for surveillance or following the disease for any signs of progression.
Palliative treatment addresses symptoms. It does not resolve or directly treat endometriosis. When endometriosis causes you severe pain, palliation may include hormone treatments, birth control pills, physical therapy, stress management plans, and diet changes.
At EndoWest, we use colleagues with other areas of specialized training. These include: pain management providers, physical therapists focused on pelvic pain relief, and other professionals to provide a multi-disciplinary treatment approach to better support your wellness.
The most effective form of endometriosis surgery is excision surgery, as it has a focus on full removal of the diseased tissue rather than just guessing how deep to burn that we see in most compared to other forms of surgery. Whether it is a chocolate cyst or endometriosis cyst in the ovary, a deep mass in the bowel, endometriosis of the appendix, bladder, or diaphragm, or deep scarring around ureters or nerves, excision of the disease is usually possible (patients are often told it is not possible or too risky).
As always, the steps you decide to take will be what you are comfortable with. We put you in control of your care. Our team will provide you with all the information you need to make a well-reasoned choice based on your unique situation and needs.
As you continue through observation or symptom control, we make sure there is a plan in place to monitor your situation and monitor the disease for progression. This may include determining whether you are able to continue working or going day to day without experiencing severe, ongoing pain. Based on the results of your care, we will make sure you can choose other potential options for managing your symptoms and disease. The choice and timing of treatment is yours.
Because palliation cannot stop the progression of endometriosis, we continue to check in with you about whether palliation is meeting your current needs based on your own life and the stage of the disease.
